CheckpointEHR Referral Program Terms & Conditions

Effective Date: September 16, 2024

1. Overview CheckpointEHR ("the Company") values referrals and offers a monetary reward to individuals or entities ("Referrers") who successfully refer new customers to the Company. This document outlines the terms and conditions governing the CheckpointEHR Referral Program ("the Program"). By participating in the Program, Referrers agree to these terms and conditions.

2. Eligibility 2.1. To qualify for the Program, Referrers must submit a referral form prior to the referred practice receiving a contract offer from the Company. 2.2. Only referrals made on or after September 16, 2024, are eligible for participation in the Program. 2.3. The referral must result in the referred practice becoming an activated CheckpointEHR customer and completing three (3) consecutive monthly payment cycles. 2.4. The Program does not apply to the following situations:

  • Staff transfers from existing customers.
  • The formation of new practices derived from existing customers.
  • Additional referrals for the same practice (where the first referring party will receive the incentive).
  • Prior customers of the Company that are returning.

3. Reward 3.1. Once the referred customer meets the eligibility criteria outlined in Section 2, the Referrer will receive a $1,000 reward. 3.2. The reward is considered the Referrer’s money and may be spent at their discretion.

4. Referral Submission 4.1. Referrers must submit referral forms through the designated method provided by CheckpointEHR. 4.2. Referral forms must include accurate and complete information about the Referrer and the referred practice.

5. Payment of Rewards 5.1. Rewards will be issued within 30 days of the referred practice completing its third monthly payment cycle. 5.2. The reward will be issued in the form determined by CheckpointEHR (e.g., check, electronic transfer).

6. General Terms 6.1. CheckpointEHR reserves the right to verify all referrals and payment cycles before issuing rewards. 6.2. CheckpointEHR reserves the right to modify or terminate the Program at any time without prior notice. Modifications or termination will not affect rewards for referrals already submitted and eligible under prior terms. 6.3. Fraudulent or unethical behavior, as determined by CheckpointEHR, will result in disqualification from the Program and forfeiture of any rewards. 6.4. The Program is void where prohibited by law.

7. Limitation of Liability CheckpointEHR is not responsible for any technical errors, delays, or failures related to the submission or processing of referral forms.

9. Acceptance of Terms By participating in the Program, Referrers acknowledge that they have read, understood, and agree to abide by these terms and conditions.
